INFORMATION SHEET to the RESERVIST REGARDING ATTACHED AF FORM712

The attached AF Form 712 has been accomplished according to current Air Force
directives and supersedes any statement of creditable retirement service furnished
to you previously.

The original copy of the AF Form 712 is filed in officer's selection folder for use
in considering him for promotion, etc. The original copy of the AF Form 712 for
the airman is filed in his master personnel record.

All service substantiated by documentary evidence in the military records, and
determined creditable for retirement, is included in the statement. You are urged
to review this document carefully. If you do not agree with the service shown it is
your responsibility to furnish documents within 60 days to substantiate your claim

for any additional service or points.

‘The anniversary date for the year of retirement is 1 July for those reservists
who were in an active status of a Reserve component on 1 July 1949, and who have
not been removed from such status.

For reservists who became members of a Reserve component or who returned to
an active status in a Reserve component after 1 July 1949, the year of service for
retirement begins on the date the person attains a Reserve status or the date of
latest return to an active status.

Public Law 86-197 extended AUS without component appointments to 31 March
1953 if not terminated sooner by administrative action or provision of law. If
you held such appointment on 1 July 1948, but did not have Reserve status, you
have been credited with a 50-point year ending 30 June 1949. Subsequently, you
are credited with 15 points yearly for membership, until 31 March 1953. Only
those reservists who did not hold a Reserve commission during this period are
affected by this adjustment.

Yearly reports of points earned will be furnished each reservist. Your copy of
the AF Form 712 should be posted from this report to keep records current, as
only one AF Form 712 will be submitted.

NOTE: The unit of assignment is forwarded reservist's copy
of AF Form 712 if it is the initial form prepared.
It should be discussed with reservist and furnished
him if he agrees with service shown thereon, or
returned with substantiating documents for correction.
